Can anyone name a fantasy book based on these few sketchy details?

Was it Terry Pratchett's "Discworld" series? It's a match for the first, and for the third if you're thinking of the sub-series about Death's apprentice and his family (which starts with "Mort"). I'm not sure that it matches the second criterion, though.
